3.1.2.1.2 Pharmacodynamics -
Quinine destroys erythrocytic schizonts (dividing stage of the protozoan
in red blood cells) in all forms of malaria in humans.
Discussions focus on whether the mechanism is based on intercalation. The
quinine molecule intercalates itself
between the bases of the parasite DNA & inhibits nucleic acid synthesis (5, 6). Vis a
vis other unicellular organisms
(e.g. bacteria, yeast) &
spermatozoa, quinine also manifests a more or less destructive effect
(6). Some of the areas of use outlined above refer to the
analgesic (pain-relieving), antipyretic
(feverreducing), local anaesthetic & muscle-relaxing effects of quinine.
The muscle-relaxing effect is based on the antagonistic action to
physostigmine on the skeletal muscles as also exhibited by curare. The
excitability of the motor end plate is reduced by quinine & reactions to
repeated nervous stimuli & acetylcholine are reduced by quinine (2, 5, 6).
On the
gravid uterus quinine has a slightly oxytocic effect. In the past quinine
was, therefore, used in the induction phase of birth at an oral dose of
between 300-500 mg twice daily in order to induce labour although this
effect has not been verified (7). It is no longer administered for this
purpose. There are numerous case reports of the ingestion of high doses of
quinine (mostly several grams) to self-induce
abortion

recently rekindled the interest of scientists & clinicians alike
because of its potent medicinal value. The olive is a hardy tree that
manufactures its own potent antibiotic substances to fend off disease
causing bacteria, fungi, parasites & insects.
In 1908, Bourquelot and Vintilesco isolated a bitter glucoside
(structurally classified as an iridoid) from olive leaves & named it oleuropein...Oleuropein
has also been found effective against strains of malaria,
including plasmodium falciparum, plasmodium vivax, plasmodium ovale &
plasmodium malariae. Malaria is a protozoan infection characterized by
fever, chills & profuse sweating, and occurs primarily in tropical
regions. Malarial infection can occur by transmission of the protozoa
parasite following the bite of an infected Anopheles mosquito. 1st the
sporozoites multiply in cells of the liver, then after a period usually
lasting 2-4 weeks, the parasite may invade the red blood cells. Next,
the merozoites may multiply before being released into the bloodstream.
Antimalarial activity was reported by clinicians in the 1850s who
administered decoctions of olive leaf to infected patients. In 1906,
scientists claimed that olive leaf extracts were superior to quinine,
the primary treatment of malaria at the time. Since malaria has
developed resistance to many of our present day drugs, clinicians may
want to consider olive leaf extract as an addition to the treatment of
malaria once again...
In 1960, scientists from Holland further isolated elenolic acid
(a monoterpene), which was eventually determined to be the chemical
constituent with the greatest activity against infectious microbes.
Later in that same decade, viral researchers at a U.S. pharmaceutical
company (Upjohn Co., Kalamazoo, MI) demonstrated that elenolic acid
evidenced remarkable inhibition of viruses and bacteria without damaging
the host cells in vitro...
The antiviral activity of elenolic acid was shown to be greatest
in an alkaline environment (pH 7.5), which is very close to the normal
human pH range of 7.35-7.45. Virucidal activity was diminished by
incubation with amino acids lysine, glycine, cysteine & histidine, and to
a lesser extent with phenylalanine, tryptophan, serine & threonine.
Because the elenolic acid could be inactivated by free amino acids
circulating in the bloodstream, research was apparently discontinued. Then
in 1994, an independent research team achieved a breakthrough that
apparently overcame this hurdle, allowing sufficient levels of elenolic
acid to be delivered to the body. Today, preparations of olive leaf
extract are standardized to contain approximately 5% elenolic acid
content, & researchers are presently working to increase that
percentage to 15%. Viruses that have shown susceptibility to elenolic
acid in vitro include: herpes (MRS); vaccinia; pseudorabies; influenza A
& B; Newcastle disease; parainfluenza 1, 2, & 3; coxsackie A21;

Volume 7) -The quintessential antimalarial lead was
quinine. Originally isolated from
Cinchona
bark, it acted as the template for the synthetic
agents of the chloroquine/mefloquine type.137
With the increase in the number of parasites resistant
to these agents came the search for other synthetic &
natural product-based agents. Inspection of data from Chinese
herbal remedies led to the investigation of extracts of
Artemisia annua
(wormwood) as this particular plant
had been used for centuries in China as an antimalarial164
(known as ‘Quinghaosu’); in 1972, the active agent was isolated & identified as a
sesquiterpene
endoperoxide named artemisinin (58;
Figure 8). Tan
et al.165
reported that this particular agent is not limited to the species
A. annua,
but is also found in at least 2 other species. Using the base
structure of artemisinin, many semi-synthetic compounds were made
with the aim of optimizing the pharmacology of the base molecule.
This led to the identification of
artemether (59;
Figure 8), a relatively simple modification of the actual
active metabolite
dihydroartemisinin (60;
Figure 8), as a potent
antimalarial agent that is now used throughout the world.

hectare, or 2.5 acres. Mosquito larvae thrive on it. 'Maize
pollen is an accelerant for mosquito populations,' McCann says.
'They just explode. You may as well be putting kerosene on a
fire.' Kebede learned that mosquitoes that feed on maize as
larvae tended to have a larger wingspan & a longer lifespan, & are therefore more likely to transmit malaria
simply because they bite more people more times...

The Ethiopian government introduced a new variety of maize,
which sheds its pollen during late August & early September,
weeks later than other varieties of maize & at precisely the
time when temperature & moisture are ideal for

One (potentially disastrous) solution would have farmers grow a
genetically modified variety of maize, whose pollen contains an
insecticide that would kill mosquito larvae... Another possible
solution would involve detasseling, removing the
pollen-producing tassel from a maize plant by hand & thus preventing pollination...
A 3rd option, he says, would be
to encourage farmers to grow a maize variety that
sheds its pollen earlier in the season, prior to
peak mosquito development. (4th) Farmers could also
plant their fields at least 60 meters from their
homesteads, he adds, to create a buffer zone between
the breeding grounds & the houses. (5th) But the most logical
solution, says McCann, is to encourage farmers to
grow local varieties of self-pollinating grains,
such as teff, or vegetables such as red pepper, near
mosquito breeding grounds."

"Mefloquine, sold under the name
Lariam,
was given to military members to protect against malaria, an infectious
disease transmitted by mosquitoes.
The drug is no longer sold in the US. But it was administered
weekly, in tablet form, to military personnel in Iraq & Afghanistan. In 2013, the
U.S. Food & Drug Administration (FDA) published a safety
alert on mefloquine & added a black-box warning for both
neurological effects & 'serious psychiatric' effects. The
side effects include: convulsions or seizures, confusion, paranoia,
hallucinations, unusual behavior, mood changes, agitation, depression,
suicidal thoughts & suicidal behavior. Mefloquine side effects can last
for months or years after the drug was stopped. It is unclear why this
drug was prescribed for military personnel when other malaria drugs with
decades of demonstrated efficacy & safety were available. Veterans may
file for disability compensation for side effects related to mefloquine
use during military service.
Mefloquine was an experimental venture between the
U.S. Department of Defense (DOD) & a pharmaceutical company. Walter Reed
Army Institute of Research conducted phase I & II clinical trials,
Hoffman-LaRoche & Smith Kline skipped phase III safety trials & the FDA
approved mefloquine in 1989. Medical professionals were reporting &
documenting severe neuropsychiatric side effects by 1994. Not until 2016
did the U.S., UK, Australian & German militaries ban Lariam because
'research showed the drug can cause permanent side effects & brain
damage.'”
