The following is one HRT protocol for a 50 year old woman.  Take hormones AM - 100 mg pregnenolone, 50 mg DHEA, Triest gel (estriol, estradiol, estrone) .6 ml days 1-10 following menses & .4 ml days 11-14, micronized progesterone gel - .6ml days 1-10 & .8 ml until menses begins, plus micronized testosterone gel at .1 ml daily.  Analogs called progestins are the result of synthetic progesterone use.  Bio-identical progesterone can be helpful for both men and women to offset lack of exercise and excess estrogen, such as found in (meat and milk) cattle that have been given hormones.  Typical estrogen ratio is 60/10/10-80/20/20 of estriol/estradiol/estrone.  Typical Triest formulation is 80/10/10.  In excess, estradiol & estrone may stimulate female cancers.  Estriol may reduce cancer. The 3 estrogens are interdependent, however.  Hormones protect the heart.

"The ovary of various animal species, serves as the body's primary source of estradiol-17 beta (E2), the steroid hormone associated with protection of the premenopausal woman from a variety of potential postmenopausal health problems, such as increased risk for decline in cardiovascular, skeletal, & genitourinary system function, & for increased incidence of hot flashes.  In addition to E2, the premenopausal ovary also secretes other steroid hormones, such as progestins & androgens, & the glycoprotein, inhibin.  Progestins, androgens & inhibin all have incompletely understood functions within the hypothalamic-pituitary-ovarian (H-P-O) axis & much less well-defined roles in the proper functioning of peripheral tissues.
The change in ovarian function across the menopause is accompanied by the loss of virtually all of the primordial & developing follicles residing within the ovary.  From a non-replenishable store of several million primordial follicles formed within the developing human ovary during gestation, approximately 25,000 follicles remain at the start of the final 12-15 years of menstrual cycling.  These remaining follicles appear to undergo an accelerated rate of loss of over twice that of the 1st 2 decades of reproductive life.  The molecular & cellular mechanisms responsible for this acceleration of follicular loss are virtually unknown.   Consequently only several hundred or less apparently gonadotropin-insensitive follicles remain in the ovary at the time of menopause, a number likely to be too small to sustain the H-P-O interactions required for regular menstrual cyclicity. 
 Follicular atresia is the predominant fate of ovarian follicles.  Of the millions of follicles initially present in the ovary, well over 99% are depleted over the woman's reproductive life span through atresia.  Except for the single ovulatory follicle per month in a normally cycling woman, all of the growing pool of antral follicles is resorbed within the ovary by atresia.  Even women not cycling due to oral contraceptive usage or pregnancy lose ovarian follicles at rates comparable to those in cycling women.  The underlying mechanism of atresia of antral follicles in diverse species, including humans, appears to involve apoptosis in various follicular cells.  The molecular & cellular mechanisms through which these apoptotic processes are regulated are just starting to be explored.
Menstrual cycles in regularly cycling middle-aged women gradually grow shorter due to a shorter follicular phase, while the length of the luteal phase generally remains constant.  As women enter the perimenopausal period, their cycles gradually lengthen, as the fraction of ovulatory cycles declines and cycles become irregular.  Early follicular phase FSH levels begin to rise about 5 years preceding the menopause, even in regularly cycling women.  Inhibin B, a potential biomarker of ovarian follicular function in the prw & peri-menopausal woman, is reported to decline in older ovulatory women with increased serum FSH levels.  Characteristics of pulsatile pituitary gonadotropin secretion driven by changes in hypothalamic GnRH secretion in women in their 40s with normal cycles & hormonal levels may explain the rise in FSH levels.  Thus, age-related changes in the H-P-O axis may to play an important role in the menopausal process.
LH levels become elevated roughly 1 year prior to menopause. Data on serum levels of E2 & progesterone suggest elevated levels of E2 & diminished levels of progesterone relative to young premenopausal women during at least part of the peri-menopause. Changes in the formation and function of the corpus luteum during the perimenopause are not well described.
Thus, menopause marks a dramatic change in ovarian function.  Prior to menopause, the ovary participates as a key component of the dynamic hormonal orchestration of the H-P-O axis resulting in the periodic release of oocytes; subsequent to menopause, the ovary functions as a relatively quiescent androgen-secreting organ in a hypergonadotropic environment.  The molecular and cellular mechanisms involved in the increased magnitude of the rise in early follicular phase FSH, the development of menstrual cycle irregularity, the rise in LH late in the menopausal process, the regulation of E2, progesterone & inhibin levels during the menopausal process.  The apparent acceleration in the rate of follicular loss early in the menopausal process are largely unknown.
By what molecular mechanisms do the menopause-related changes in the aging H-P-O axis interact with non-reproductive tissues leading to the health problems & conditions associated with peri & post-menopausal women? 
Clearly the loss of ovarian estrogen is a major factor involved in this process, as we learn that many non-reproductive tissues are responsive to protection by estrogen.  Increased estrogen levels during the perimenopause coupled with declining progesterone levels may explain abnormal uterine bleeding & perhaps the development of uterine fibroids.  Several non-reproductive somatic tissues associated with postmenopausal health problems, such as bone & cardiovascular tissue, make their own supply of estrogen via endogenous estrogen synthetase (aromatase) &/or activate estrogen within the tissue via endogenous 17-beta hydroxysteroid dehydrogenase activity.
Other H-P-O axis factors may also be involved.  For example, the observations that a) gonadotropin levels are increased in estrogen-deficient postmenopausal women, b) glycosylated forms of serum LH and FSH are altered in the postmenopause, and c) gonadotropin receptors have been detected in non-reproductive tissue, increase the possibility that these altered postmenopausal levels & forms of gonadotropins may have physiologic or pathophysiologic roles outside of the reproductive system.  Also, loss of gonadal inhibin in peri & post-menopausal women may affect osteoblastogenesis & osteoclastogenesis, thereby contributing to bone loss during & subsequent to the menopause.   The frequently greater complaint of menopausal symptoms in surgically menopausal women suggests either the severity is related to the abrupt loss of ovarian estrogen or the postmenopausal ovary may secrete protective factors to modulate symptoms. 
Still another largely unexplored possibility is age-related alteration in tissue sensitivity to estrogen &/or other bioregulatory factors secreted by the H-P-O axis."

  2 hormones known in the female as follicle stimulating hormone (FSH) and corpus luteum stimulating hormone (LSH) are secreted by the anterior lobe of tile pituitary gland. These hormones are real gonadotropilins because they directly govern the function of the ovaries. The anterior pituitary is in turn governed by the diencephalon, & so when there is an ovarian deficiency the diencephalic center concerned is hard put to correct matters by increasing the secretion from the anterior pituitary of FSH or LSH, as the case may be. When sexual deficiency is clinically present, this is a sign that the diencephalic center concerned is unable, in spite of maximal exertion, to cope with the demand for anterior pituitary stimulation. When then the administration of HCG increases the functional capacity of the diencephalon, all demands can be fully satisfied & the sex deficiency is corrected.
That this is the true mechanism underlying the presumed gonadotrophic action of HCG is confirmed by the (act that when the pituitary gland of infantile rats is removed before they are given HCG, the latter has no effect on their sex-glands. HCG cannot therefore have a direct sex gland stimulating action like that of the anterior pituitary gonadotrophins, as FSH and LSH are justly called. The latter are entirely different substances from that which can be extracted from pregnancy urine and which, unfortunately, is called chorionic gonadotiophin. It would he no more clumsy, and certainly far more appropriate, if HCG were henceforth called chorionic dienccphalotrophin.
It cannot he sufficiently emphasized that HCG is not sex-hormone, that its action is identical in men, women, children & in those cases in which the sex-glands no longer function owing to old age or their surgical removal. The only sexual change it can bring about after puberty is an improvement of a pre-existing deficiency. But never stimulation beyond the normal in an indirect way via the anterior pituitary HCG regulates menstruation & facilitates conception, but it never virilizes a woman or feminizes a man.
It neither makes men grow breasts nor does it interfere with their virility, though where this was deficient it may improve it. It never makes women grow a beard or develop a gruff voice. I have stressed this point only for the sake of my lay readers, because, it is our daily experience that when patients heat the word hormone they immediately jump to the conclusion that this must have something to do with the sex-sphere. They ate not accustomed as we are, to think thyroid insulin, cortisone, adrenalin etc, as hormones.
